MEMBERSHIP APPLICATION
for Individual Membership
 
The European Trauma Society is furthering traumatology in general and disseminating interest in, and knowledge of, traumatology in particular.

All those who, in the exercise of their daily tasks, are actively involved in the issue of traumatology, and who share the goals of the society, are invited to join ETS.

All members of trauma societies, that are member societies of the ETS (altogether 10 national trauma societies) are members of the European Trauma Society. These 10 members are: the Austrian, Belgian, Croatian, Czech, Dutch, German, Hungarian, Slovakian, Slovenian and Swiss Trauma Society.
 
Preconditions for
Individual Membership:
 
The applicant has to
-   maintain the highest professional, moral, and ethical standards
-   comply with the dues, fees, and assessment requirements established by the Board of Directors of the European Trauma Society (see below)
-   be sponsored by two members of the European Trauma Society. Each sponsor must provide a written letter of recommendation
-   send her/his scientific curriculum to the Board of Directors for examination
-   register as such with the secretary of the Board of Directors.
 
Fees:
Individual members of the European Trauma Society will receive the European Journal of Trauma (the official publication of the European Trauma Society since January 1, 2004) as part of their membership. They receive the Journal at a
 
reduced annual subscription rate of EUR 60.-
plus shipping EUR 20.-
plus ETS-fee EUR 30.-

per year EUR 110,-
 
 
Deadlines:
The European Trauma Society will contact you directly regarding acceptance within three months of receipt of the completed form.

Membership (and subscription of the European Journal of Trauma) is always for the duration of a full calendar year. New subscriptions and delivery of the Journal is retroactively applied to January 1st of any given calendar year.

Membership applications received before October 1st will be made effective as of 1st January of the current year.

Membership applications received after October 1st will be made effective as of 1st January of the following year.

Your application must be accompanied by:
a)   A signed copy of your current scientific curriculum vitae
b)   A list of your publications
c)   A recent photograph
d)   Two letters of recommendation.
The application cannot be processed until the letters of recommendation of two ETS members and all other requested items are assembled.
